$10 Service Fee not included and must be paid at time of appointment. Must mention you have an eLiveLife Certificate when making the appointment. Must have Certificate with you at time of appointment. No cash value. No refunds, exchange only. To reschedule or cancel you must give 24 hours notice or services will be forfeited. Offer expires 6 months from date of purchase. New clients only. Valid only Sunday-Thursday. Not valid on Holiday weekends (ie Mother's Day Weekend Fri-sun). Must have individual Revive certificate to book appointment. Must leave credit card # and certificate # to book appointment. 24 hour cancellation policy to avoid $20 fee plus a voided certificate.
